Job summary

The Hospice of Windsor and Essex County is seeking a Registered Practical Nurse (RPN) to deliver compassionate, patient‑centered end‑of‑life care. You will assess, plan, implement, and coordinate care with an interdisciplinary team in Hospice residences or at the patient’s home, while mentoring staff and ensuring policy‑driven practice.

Flexibility to work days, nights, weekends, and holidays is required; prior palliative care experience is preferred.

Qualifications

• Legally entitled to work in Canada and hold CNO‑registered RPN status.
• Registration in good standing with the College of Nurses of Ontario.
• Strong communication and teamwork in a palliative care setting.

Responsibilities

• Provide direct, end‑of‑life care aligned with CHPCA domains.
• Develop and evaluate individualized care plans.
• Collaborate with physicians, RNs, PSWs and social workers.
• Mentor new staff and maintain clinical competence.
• Maintain accurate documentation and adhere to policies.
