Job summary

Spectrum Health Care seeks a Home Support Supervisor to lead a team of Personal Support Workers in the field, providing ongoing training, coaching, and performance management. This role involves travel to patients’ homes in North York, Toronto, and collaboration with multidisciplinary teams to implement care plans.

Applicants should be a Registered Practical Nurse with Ontario registration and demonstrate strong communication and leadership abilities, plus a valid driver’s license and reliable

Qualifications

• Current registration with the Colleges of Nurses of Ontario (CNO) in good standing.
• Minimum three years’ progressive leadership responsibilities as a direct supervisor in a community setting.
• Valid driver’s license and access to a reliable vehicle.
• Excellent English verbal and written communication skills.
• Solid knowledge of community nursing and service delivery to patients.

Responsibilities

• Travel to patients' homes in the community.
• Collaborate with multidisciplinary teams in a community setting.
• Assess patient needs, formulate the plan of care, and assess progress toward outcomes.
• Manage staff, conduct performance appraisals, and implement progressive discipline.
• Gather, analyze patient and staff feedback and monitor key indicators (caseload, turnover, incidents, satisfaction, risks).
• Address and document patient concerns and complaints promptly.
