Job summary

Spectrum Health Care (SHC) is seeking Registered Practical Nurses (RPN) as Home Support Supervisors to lead a team of Personal Support Workers across Peel, North York, Scarborough, York, and Toronto. The role focuses on training, coaching, and performance management in a community setting.

You will travel to patients' homes, collaborate with multidisciplinary teams, and monitor outcomes and patient feedback to ensure high-quality care and service delivery.

Qualifications

• Current registration with the Colleges of Nurses of Ontario (CNO) in good standing.
• Minimum three years' experience with progressive leadership responsibilities as a direct supervisor, preferably in a community setting.
• Valid driver's license and access to a reliable vehicle.
• Exceptional interpersonal skills and ability to handle difficult situations objectively.
• Excellent English verbal and written communication skills.
• Solid knowledge of the principles, practices, and methods of community nursing and service delivery to patients as well as program development, implementation, and evaluation.

Responsibilities

• Ensures that all patient concerns and complaints are addressed and documented in a timely manner.
