Job summary

Ontario Health atHome in North Bay is seeking a Care Coordinator to assess patient needs, determine eligibility for services and connect patients with community resources. You will work with patients, families and a multidisciplinary team to deliver connected, patient-centred care.

Requirements include regulatory body membership in good standing (Nursing, Physiotherapy, Occupational Therapy or Social Work in Ontario), recent community health experience, strong communication and problem-solving

Qualifications

• Membership in good standing with Ontario regulatory bodies (Nursing, Physiotherapy, Occupational Therapy, Social Work).
• Recent experience in community health or related field.
• Knowledge of the health care delivery system and community resources.
• Excellent interpersonal, communication, assessment, problem-solving and decision-making skills.
• Effective time management, prioritization and organizational skills.
• Ability to complete required documentation and forms.
• Valid driver’s license with access to a reliable vehicle.
• Proficient in Windows.
• Advanced English and French oral and written proficiency.
• COVID-19 vaccination status required.
• Diversity experience; case management; second language a plus.

Responsibilities

• Assess care needs with patients and families and determine service eligibility.
• Link patients with service providers and coordinate care plans.
• Monitor delivery of care plans and adjust as needed.
• Establish trustful relationships with patients and families.
• Balance needs with resources while respecting preferences.
• Initial hospital assignment with possible changes based on needs.
