Job summary

CBI Home Health is seeking a Speech Language Pathologist to join our Hamilton and Oakville Halton region teams, delivering high-quality care in clients’ homes. You’ll assess, treat and support communication and swallowing needs with a patient- and family-centered approach, integrating digital health tools.

You must be in good standing with the College of Audiologists and Speech Language Pathologists of Ontario, hold a valid driver’s license, and enjoy a flexible schedule with opportunities for

Qualifications

Registered as a member in good standing with the College of Audiologists and Speech Language Pathologists of Ontario.
Valid driver’s license and vehicle insurance.
Able to deliver care in person and via virtual methods.
Strong communication and collaboration within an interdisciplinary team.

Responsibilities

Assess and implement SLP interventions (swallowing, dysphagia, communication) in a home-based setting.
Use a collaborative, interdisciplinary team-based approach to care.
Support holistic care including stroke pathway and cognitive communication supports.
Leverage digital health platforms for care delivery, documentation and intervention.
