Job summary

CBI Home Health seeks a Speech Language Pathologist to deliver home-based SLP services in the Durham region, focusing on swallowing, communication, and cognitive supports. You will collaborate with a multidisciplinary team to provide personalized, compassionate care in patients' homes.

The role requires current Ontario SLP registration, a valid driver’s license, and ability to travel within Durham. A flexible schedule supports work-life balance and patient-centered outcomes.

Qualifications

• Registered with the College of Audiologists and Speech Language Pathologists of Ontario.
• Valid driver's license and vehicle insurance required.
• Experience in home-based care or community settings preferred.

Responsibilities

• Assess and implement SLP interventions in home-based settings.
• Work with an interdisciplinary care team to plan services.
• Support restorative care and dysphagia management.
• Leverage digital health tools for documentation and care delivery.
