Job summary

CBI Home Health is seeking a Speech Language Pathologist to provide assessments and interventions in home-based settings across Brant, Haldimand, and Norfolk. The role emphasizes collaborative, interdisciplinary team care and a holistic approach to swallowing, communication, and cognitive supports.

You will use digital health platforms to document and coordinate care, and participate in flexible scheduling with opportunities for growth within the national network of CBI Health.

Qualifications

• Active registration with College of Audiologists and Speech-Language Pathologists of Ontario.
• Valid driver's license and vehicle insurance.
• Ability to work in home-based and virtual settings.
• Strong communication and collaborative/interdisciplinary teamwork.
• Patient-centered care mindset.

Salary Range: The salary range for this position is $91.14 – $100.25 per visit

Sign on Bonus: $2,000*

What you will do

• Assessment and implementation of SLP interventions (i.e. swallowing assessments, dysphagia management, communication, etc.) in a home-based setting
• Use of a collaborative and interdisciplinary team-based approach to care and services
• Support a holistic and restorative approach to care (i.e. enhanced stroke pathway; cognitive communication supports, etc. )
• Leverage our digital health platforms to support care delivery, collaboration, documentation, and intervention

What you will need to be successful

• You are registered as a member in good standing with the College of Audiologists and Speech Language Pathologists of Ontario
• Valid driver's license and vehicle insurance
• Your focus is to embrace person and family-centered care
• You are comfortable delivering care both in person and using virtual methods/technology
• You are passionate, empathetic, use good judgement and apply critical thinking when making client decisions
• You are known for your excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
• You are flexible and have demonstrated skills in critical thinking that you can apply to a variety of situations.
• You value the role a therapist plays in keeping clients at home, where they want to be
