Job summary

CBI Home Health is seeking a Speech Language Pathologist to join our Hamilton and Oakville Halton region teams, delivering high-quality care in clients’ homes. You’ll assess, treat and support communication and swallowing needs with a patient- and family-centered approach, integrating digital health tools.

You must be in good standing with the College of Audiologists and Speech Language Pathologists of Ontario, hold a valid driver’s license, and enjoy a flexible schedule with opportunities for

Qualifications

• Registered as a member in good standing with the College of Audiologists and Speech Language Pathologists of Ontario.
• Valid driver’s license and vehicle insurance.
• Able to deliver care in person and via virtual methods.
• Strong communication and collaboration within an interdisciplinary team.

Responsibilities

• Assess and implement SLP interventions (swallowing, dysphagia, communication) in a home-based setting.
• Use a collaborative, interdisciplinary team-based approach to care.
• Support holistic care including stroke pathway and cognitive communication supports.
• Leverage digital health platforms for care delivery, documentation and intervention.

Salary Range: The salary range for this position is $91.14 - $100.25 per visit

Sign on Bonus: $2,000*

What you will do

• Assessment and implementation of SLP interventions (i.e. swallowing assessments, dysphagia management, communication, etc.) in a home-based setting
• Use of a collaborative and interdisciplinary team-based approach to care and services
• Support a holistic and restorative approach to care (i.e. enhanced stroke pathway; cognitive communication supports, etc. )
• Leverage our digital health platforms to support care delivery, collaboration, documentation, and intervention

What you will need to be successful

• You are registered as a member in good standing with the College of Audiologists and Speech Language Pathologists of Ontario
• Valid driver's license and vehicle insurance
• Your focus is to embrace person and family-centered care
• You are comfortable delivering care both in person and using virtual methods/technology
• You are passionate, empathetic, use good judgement and apply critical thinking when making client decisions
• You are known for your excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
• You are flexible and have demonstrated skills in critical thinking that you can apply to a variety of situations.
• You value the role a therapist plays in keeping clients at home, where they want to be
