Job summary

Chartwell Retirement Residences seeks a Health and Wellness Manager to lead a care team and deliver personalized support that makes residents feel valued and at home.

You will collaborate with residents, families and other departments to ensure dignified wellness journeys, while coaching nurses and staff. A valid nursing license and strong documentation skills are essential.

Qualifications

• Minimum 2 years clinical experience.
• Leadership or supervisory experience in healthcare or retirement settings is a plus.
• Licensed RN or RPN in good standing with provincial college.
• Proficient in clinical documentation and resident care plans.

Responsibilities

• Lead, supervise and support wellness team members.
• Ensure appropriate care levels based on resident needs.
• Use nursing process to guide care and wellness plans.
• Complete admissions, assessments, discharges, and progress notes.
• Collaborate with residents and families in care decisions.
• Contribute to safety and compliance through DO duties.

Health and Wellness Manager

You’ll lead a dedicated care team to deliver personalized support that makes residents feel valued, secure, and truly at home. You’ll be part of a collaborative management team, working together to ensure every resident’s wellness journey is dignified, engaging, and meaningful.

What You’ll Do

• Ensure residents receive the right level of care based on individual needs
• Use the full nursing process (assess, plan, implement, evaluate) to guide care delivery and wellness plans
• Complete required clinical documentation, including resident admissions, assessments, discharges, and progress notes
• Partner with residents and families in making informed care decisions
• Lead, support, and supervise Nurses and other wellness team members
• Respond promptly and professionally to residents and family concerns
• Collaborate across departments to ensure smooth daily operations and step into the Manager on Duty role
• Participate in our Joint Occupational Health & Safety Committee and champion safe work practices

What You’ll Bring

• Minimum 2 years clinical experience
• Experience in a leadership or supervisory role in healthcare or retirement settings is an asset
• Registered Practical Nurse (RPN/LPN) or Registered Nurse (RN) in good standing with the provincial College of Nurses
• Proficiency in clinical documentation and understanding of resident care plans
• Computer literate, including Microsoft Word and Excel
• Strong organizational, communication, and problem-solving skills
• A collaborative and empathetic leadership style
• Valid Driver’s License, or reliable means of transportation

  • May require carrying a company-issued phone and working flexible hours, including weekends
  • Some travel may be required
