POSITION SUMMARY:

The Health Care Aide provides direct client care and environmental support within established standards of care, policies and procedures. The Health Care Aide functions in meeting the special and changing needs of clients and assisting them in reaching and maintaining optimum health and independence within the limits of their abilities under the direction of the team leader.

RESPONSIBILITIES:

Overview:

1. Assist clients in activities of daily living, striving to maintain self-respect, dignity, and independence by encouraging and allowing clients to care for themselves within their abilities, integrating the resident Bill of Rights (Long Term Care) into care.
2. Follow individualized care plans, provide feedback, and communicate/report client changes to the team leader.
3. Document care according to policy, including observations and care provided in the client’s health record and flow sheets.
4. Assist with personal hygiene, elimination, mobility, and other care activities as per care plan, ensuring safety and dignity.
5. Provide or assist with observation, measurements, safety measures, and documentation as per policies.
6. Assist with application and care of medical devices, oxygen therapy, and medication safety principles.
7. Support client admission, transfer, discharge, and post-death care, including valuables management.
8. Maintain infection control, a safe environment, and proper handling of specimens.
9. Order and stock supplies, encourage participation in activities, and demonstrate safe use of medical equipment.
10. Perform other duties as assigned.

JOB INFORMATION

Employment Status: Part Time Indefinite. Start date: To be determined.

Note: This indefinite term may expire with 24 hours' notice of incumbent's return.

Shift: Days/Evenings, 7.75-hour shifts.
