Position Summary

The Health Care Aide provides direct client care and environmental support within established standards, policies, and procedures. They meet the changing needs of clients, assisting them in achieving and maintaining optimal health and independence within their abilities under the supervision of the team leader.

Responsibilities

Overview:

• Assist clients with activities of daily living, promoting self-respect, dignity, and independence by encouraging self-care within their abilities as documented in the care plan.
• Communicate client condition changes to the team leader.
• Document care provided according to policy.
• Participate in client care conferences and in developing individual care plans.
• Assist with personal hygiene, including bathing, grooming, skin and oral care, and dressing, as per care plan.
• Assist with elimination needs, including toileting, changing incontinence products, and perineal care.
• Support mobility, including assisting with transfers, walking, and exercise programs, using proper techniques and equipment.
• Monitor and record client observations and measurements, reporting changes and updating records accordingly.
• Implement safety measures, manage disoriented or aggressive clients, respond promptly to call lights, and report incidents.
• Maintain privacy, confidentiality, and respect for clients and their families.
• Assist with admissions, transfers, discharges, and post-mortem care, including handling valuables.
• Ensure a clean and safe environment, including linen changes, spill cleanup, equipment safety, and supply management.
• Encourage client participation in activities and organize leisure activities during evenings and weekends.
• Perform other duties as assigned.
