Position Summary

The Health Care Aide provides direct client care and environmental support within established standards of care, policies, and procedures. They meet the changing needs of clients and assist them in reaching and maintaining health and independence under the direction of the team leader.

Responsibilities

Overview:

• Assist clients with activities of daily living, promoting self-respect, dignity, and independence, integrating the resident Bill of Rights into care.
• Follow individualized care plans, provide feedback, and communicate client changes to the team leader.
• Document care and observations according to policy.
• Assist with personal hygiene, elimination, mobility, and other care tasks as per care plan.
• Maintain safety and infection control standards.
• Support client participation in activities and facilitate leisure activities.
• Other duties as assigned.
