Qualifications

Bachelor’s degree in Social Work from an accredited university.

Minimum two years’ healthcare experience; recent work with older adults preferred.

Responsibilities

Provide specialized assessment, treatment, and intervention for adults with multiple complex health needs.
Educate and mentor learners; participate in education and training as required.
Develop and implement clinical procedures and specialty programs.
Collaborate with clients, families and interprofessional teams to meet client needs.
Maintain relevant statistical information and monitor outcomes.
