Job summary

SE Health in Brampton and Caledon seeks a Social Worker to provide person-centered care and assess clients in the community, collaborating with an interdisciplinary team to support physical, cognitive, social, and emotional well-being.

The role requires a Master’s of Social Work and active registration with the Ontario College of Social Workers and Social Service Workers, with preferred experience in acute/rehab or community rehab. Flexible community-based work with opportunities to grow.

Qualifications

• MSW degree and active Ontario college registration.
• Experience in acute/rehab or community rehab preferred.
• Reliable, flexible, self-directed professional, able to work with an interdisciplinary team.
• Knowledge of community resources is a plus.
• Strong interpersonal, organizational, and communication skills.
• Positive attendance record and adaptability to new computer applications.
