Job summary

CAMH is seeking a part-time, permanent Social Worker 2 for the Complex Care and Recovery Program in Toronto. The role involves psychosocial assessments, individual and group counselling, and discharge planning for clients aged 65 and over with mental health and addiction issues.

You will work across outpatient and inpatient teams, liaising with community agencies and teaching other professionals. A Vulnerable Sector check is required.

Qualifications

• Experience with geriatric clients and knowledge of aging processes is an asset.

Job Description

The Geriatric Mental Health Outpatient and Inpatient Services is currently seeking a part time, permanent Social Worker 2 to work in the Complex Care and Recovery Program. Reporting to the manager of Geriatric Mental Health Services, you will be expected to work collaboratively within an interprofessional team setting. Your target population of clients will be 65 years of age and older, with mental health and addiction issues, often with co-occurring medical issues.

Responsibilities

Your time will be divided between the outpatient team and inpatient team. It will include the provision of in-depth psychosocial assessments, along with other clinical assessments, with patients and families and will offer appropriate interventions and recommendations through the development of integrated care plans. You will provide or co-facilitate a combination of group and individual counselling sessions to clients, as appropriate, and collaborate with the interprofessional clinical team in the formulation of individualized client care plans. You will also provide education and financial or social supports/assistance to clients, family and caregivers from diverse backgrounds. You will play a primary role in the client’s discharge planning activities – particularly in addressing housing, income and aftercare needs. As such, you will demonstrate your expertise in liaising with community agencies and established community partners. The successful candidate will leverage their strong coordination, communication and collaboration skills to facilitate successful follow-up in the community. You will be expected to participate in program planning, program evaluation, quality improvement, and research activities. Candidates must be capable of providing supervision to Masters of Social Work students, collaborating with faculties of social work on special projects related to the development of the social work field, contributing to the development of best practices unique to the social work field and also teaching to other professionals. Qualifications

The successful candidate will possess a Master of Social Work degree and be registered and a member in good standing with the Ontario College of Social Workers and Social Service Workers. Proven experience in Social Work with geriatric clients, knowledge of the aging process and knowledge of geriatric care issues is an asset. The ability to provide culturally competent services to clients of diverse ethno-racial and cultural backgrounds is essential and experience working in a multi-system, inter-professional environment is an asset. Excellent leadership, organizational, interpersonal and written communication skills and the ability to work effectively in a fast-paced clinical environment are also essential. Computer skills, including word processing, electronic communication and workload measurement are essential. Bilingualism (French/English) or proficiency in a second language would be an asset. The offer of employment will be conditional upon receipt of the satisfactory Vulnerable Sector Police Records Check.

Salary Range

$40.25-$54.24/hour
