Job summary

Circle of Care, Sinai Health is seeking a Full-Time Community Social Worker on a 12-month contract to collaborate with hospital and community partners, providing linkages with rehab options, home and community support services, geriatric care navigation, and caregiver support.

Based at Head Office with regular community travel, this role reports to the Manager, Social Work, ADP, Hospice and Bereavement and offers a salary range of $65,000 to $76,500 per year, depending on experience.

Qualifications

• MSW or BSW degree with healthcare focus; must be registered with the Ontario College of Social Workers and Social Service Workers.
• Registered and in good standing with the Ontario College of Social Workers and Social Service Workers.
• 2+ years of experience in a healthcare client service environment.
• Clinical experience in community care, home care, or case management.
• Understanding of gerontological and mental health issues, especially with seniors.
• Knowledge of community resources, health care, and social service systems.
• Strong advocacy, person-centered focus, decision making, problem solving, negotiation, and conflict resolution skills.
• Excellent interpersonal and communication skills.
• Strong client and customer focus.
• Proven bias for action, excellent critical thinking and time management skills.
• Excel and PowerPoint proficiency.
• Ability to travel to various worksites; access to a vehicle.
• Ability to provide field educational and practicum supervision.

Responsibilities

• Ensure clients and families experience high quality, responsive social work service.
• Collaborate with hospital inter-professional teams, OHAH, Care Coordinators, and community support roles.
• Provide information, advice, and support on the plan of care.
• Assess for additional home and community services beyond current provisions.
• Review triage referrals with the team.
• Continuously assess client needs and adjust the care plan.
• Provide short-term counselling to clients/caregivers and initiate community linkages.
• Coordinate and facilitate diverse support groups to address needs and promote mutual support.
• Advocate to ensure timely, appropriate services.
• Lead or participate in consultations and case conferences with providers.
