Job summary

St. Michael’s Health Centre, part of Covenant Health, is seeking a Licensed Practical Nurse to join the Seniors Care 1West unit. The LPN will provide holistic, person-centred care in a multidisciplinary team, supporting residents and their families in a compassionate, culturally safe environment.

Requirements include Practical Nursing certification with CLHA registration, BLS Level C, and at least 1 year in long-term care. Experience with EHR and LTC assessments is preferred.

Qualifications

• Graduate of a Practical Nursing program; active registration with CLHA.
• Basic Life Support Healthcare Provider (Level C) required.
• Minimum of 1 year of experience in a long-term care or geriatric setting.
• Strong clinical, communication, empathy and teamwork skills.
• Must be able to understand written and verbal directions in English.
• Preferred: Geriatric/LTC nursing certification (e.g., CNLTC); training in crisis management (NCI/TCI); EHR experience (Connect Care); InterRAI MDS/LTCF assessments.

Responsibilities

• Provide holistic nursing care within a multidisciplinary team.
• Support post-acute rehab, dementia and palliative care in a nurse-led model.
• Perform nursing tasks with accuracy, compassion, and safety.
• Coordinate care activities and contribute to quality improvement initiatives.
• Communicate effectively with residents, families, and care team.
