Job summary

Fraser Health is seeking a Registered Nurse to provide direct patient care for acutely ill elderly patients in a hospital setting. You will assess, plan, implement, and evaluate nursing care in collaboration with a multidisciplinary team.

You will contribute to discharge planning, patient education, and care coordination while adhering to BCCNM standards and Fraser Health values, including respect, caring, and trust.

Qualifications

• Graduation from an approved school of Nursing and current practice registration with BCCNM.
• One year recent related acute care experience or equivalent.
• Ability to assess clinical and diagnostic data and plan nursing care.
• Excellent communication with patients, families, and care teams.
• Commitment to patient-centered care and cultural safety.

Responsibilities

• Assess acutely ill elderly patients in an acute care unit.
• Develop and modify care plans with patients and families.
• Collaborate with interprofessional care team for comprehensive planning.
• Coordinate nursing care, delegating tasks per protocols.
• Provide patient education and discharge planning.
• Document assessments, interventions, and outcomes.
