Job summary

Saint Elizabeth is seeking a dedicated nurse in Ontario to provide high-quality home and community care on a per-visit pay model. You will assess clients, develop care plans, and lead care within your caseload while collaborating with a multidisciplinary team.

The role requires registration with the College of Nurses of Ontario, CPR certification, and a valid driver's license with a willingness to obtain Saint Elizabeth certifications.

Qualifications

• Current registration with the College of Nurses of Ontario is required.
• CPR certificate is required.
• A valid driver’s license and vehicle (travel required).
• Updated immunization records and background checks are required upon hire.
• Willingness to attain Saint Elizabeth certifications.
• Availability to workdays, evenings, nights and every other weekend.
• This is a physically demanding position with lifting up to 55 pounds.

Responsibilities

• Complete comprehensive client assessments and develop care plans.
• Act as primary provider for a client caseload and coordinate with care team.
• Support clients through transitions by connecting with community services.
