Hospice Nurse Practitioner - PRN

Responsibilities

• Communicate significant findings and changes to the clinical team
• Complete face-to-face encounters assessing disease changes, functional decline, pain and symptom management, and quality of life
• Order and review labs, x-rays, EKGs, and other diagnostics; document treatment plans and review results with physicians
• Make referrals to other disciplines as needed
• Participate actively in interdisciplinary team meetings

Qualifications

• Current, unencumbered Nurse Practitioner license in the state of employment
• At least one year of experience as a Nurse Practitioner or hospice RN within the last three years
• Authority to practice and prescribe as a nurse practitioner in the state of service
• Medicare billing privileges
• Compliance with state-specific physician-nurse practitioner collaborative requirements
• State-required insurance for practice and prescribing
• Knowledge of Medicare, Medicaid, and other regulatory requirements
• Current CPR certification
