The Community Outreach Nurse will play a vital role in SRx Health Solutions' efforts to expand access to care for individuals at risk of or living with Hepatitis C (HCV) and HIV. The role involves community-based support, including point-of-care (POC) testing, patient engagement, care coordination, and developing new testing sites.
Location: Lethbridge, Alberta (and surrounding areas)
What You Will Be Doing
Hepatitis C Virus (HCV) and HIV Point-of-Care Testing:
- Conduct POC and confirmatory serum testing in various community settings, outreach locations, or mobile units.
- Document test results and patient data in compliance with clinical guidelines and confidentiality standards (using HepSYNC).
- Provide counseling on HCV/HIV, risk reduction strategies, and treatment options.
Patient Engagement And Support
- Establish trust with patients to encourage care engagement.
- Facilitate referrals for confirmatory testing, treatment initiation, and follow-up.
- Support patients throughout treatment, ensuring attendance at appointments and adherence to therapy plans.
Locating And Re-engaging Patients
- Collaborate with healthcare providers, harm reduction teams, and social services to locate and reconnect with patients lost to follow-up.
- Use community outreach strategies to re-engage patients in HCV/HIV care and treatment.
Coordination Of New Testing Sites
- Identify and establish three new community-based testing sites per quarter in underserved or high-risk areas.
- Partner with local organizations to manage these sites effectively.
- Ensure appropriate staffing, resource allocation, and logistical support for testing operations.
Care Coordination
- Navigate patients through the healthcare system by assisting with medical appointments, medication access, and treatment understanding.
- Address social determinants of health, such as housing and substance use, through collaboration with social workers and other specialists.
Community Engagement And Advocacy
- Build and maintain relationships with community organizations and harm reduction services to enhance outreach efforts.
- Advocate for reducing barriers to HCV/HIV care, particularly for marginalized populations, including those experiencing homelessness or substance use.
Data Collection And Reporting
- Record patient encounters, test results, follow-ups, and outcomes in HepSYNC.
- Track performance metrics and contribute to evaluating program impact.
About Your Qualifications
- LPN (Alberta)
- Certification in phlebotomy required.
- Experience in community outreach, public health, or a related healthcare field an asset.
- Previous experience in Hepatitis C and HIV care, harm reduction, or infectious disease management is an asset.
- Strong communication and interpersonal skills for building trust with patients and collaborating with multidisciplinary teams.
- Valid driver's license and access to a reliable vehicle for travel within Lethbridge and surrounding areas.
- Willingness to work flexible hours, including evenings and weekends, to meet community outreach needs.
- Empathy and cultural sensitivity to work effectively with diverse populations.
- Strong organizational and problem-solving skills to manage complex patient cases and establish new outreach sites.
- Commitment to health equity and advocacy for vulnerable communities.
