Job Description

Partners Community Health (PCH) is a new not-for-profit organization focused on bringing healthcare services together around the needs of people living in Mississauga and West Toronto. PCH operates two state-of-the-art LTC homes and a community hub in West Mississauga, with a combined 632 beds. The organization aims to introduce innovative, inclusive programs and new models of care delivery, building partnerships that prioritize people and fostering a learning healthcare community that provides excellent service across the aging continuum.

Position Summary:

PCH is seeking a Director of Care to support the Partners Community Health team. Reporting to the Executive Director, the Director of Care will ensure efficient operation of the home and high-quality resident care. You will lead a team of clinical professionals, build relationships with residents and families, and promote a culture focused on resident care and well-being.

Key Responsibilities:

  • Ensure clinical care processes are delivered according to individual care plans
  • Implement and monitor clinical programs such as falls prevention, skin and wound care, continence care, and pain management
  • Ensure compliance with regulations, policies, and legislations
  • Promote resident-centered care involving residents, families, and staff
  • Create a positive work environment for team members and external providers
  • Manage nursing expenditures, explain variances, and collaborate on corrective actions
  • Participate in strategic planning and accreditation activities
  • Establish and monitor nursing practices in line with standards and policies
  • Oversee staffing patterns and schedules within budget and agreements
  • Coordinate education and professional development for staff
  • Evaluate clinical programs and quality committees
  • Ensure compliance with health and safety policies and legislation
  • Perform other duties as assigned

Qualifications:

  • Bachelor of Nursing Science or equivalent; GNC certification is an asset
  • Current registration as a Registered Nurse with the provincial College of Nurses
  • Minimum three years of RN experience in long-term care in Ontario
  • At least three years of managerial or supervisory experience in healthcare
  • Proven leadership and customer service skills
  • Strong communication skills, including experience with documentation systems
