Located in the heart of Toronto’s west-end Roncesvalles community, Copernicus Lodge provides a continuum of care tailored to meet the evolving needs of seniors within a predominantly Polish, Roman Catholic environment.
Position Summary:
The Director of Care and Interprofessional Practice is responsible for the overall operation of the nursing department. He/she is responsible for: leading, organizing and managing the nursing and personal care services within the home while ensuring that legislative requirements are met and adhered to; ongoing assessments, monitoring delivery of care, auditing, assisting in the planning, organizing, coordinating, implementing and continued evaluation of departmental policies and procedures and human resource management, budget management, strategic planning and accreditation activities, and quality assurance and risk management projects.
Position Summary:
The Director of Care oversees all clinical and administrative operations of nursing and personal care services, as well as the scheduling department, SSW/Placement, Social Worker services, health professionals practice, and Infection Prevention and Control program of the home.
Main Duties and Responsibilities:
- Plans, organizes, coordinates, directs, and monitors the effectiveness of all operational activities within the nursing department, placement services, social work and scheduling departments.
- Promotes a resident-centered care approach that engages residents, families, and team members to deliver care that meets individual resident needs
- Models and fosters an environment of positive customer service in all aspects of care
- Promotes a positive working environment for team members and external providers
- Leads the development of the annual nursing budget
- Establishes and implements goals, objectives and policies that follow regulatory requirements and are aligned with the organization’s values.
- Promotes a positive and safe work environment for staff and residents
- Ensures that all Nursing staff function within their scope of practice and demonstrate knowledge and skills required for the work
- Ensures professional practice standards are effectively deployed, communicated, and monitored.
- Sets annual departmental goals and objectives that are congruent with the mission of the Home;
- Maintains records in accordance with the standards and guidelines of the RAI-MDS, Fixing Long Term Care Homes Act’2021 , and relevant professional standards.
- Establishes systems to ensure all standards and regulations, such as those outlined in the FLTCA’2021 and O. Regulation 246/22 or CNO Standards, are adhered to on an ongoing basis
- Ensures that all nursing committees are functioning within their terms of reference and areas of responsibility i.e., Falls, Continence, Nursing Practice, Wound Management, Infection Control, PAC
- Participates in strategic planning, accreditation activities, and committees as requested, and attends Board of Director meetings
- Participates in and leads the recruitment, orientation, and performance management process for all employees under his/her responsibility
- Establishes, monitors and evaluates staffing patterns, work schedules and assignments that maximize continuity and consistency of care, and nursing retention in accordance with the nursing envelope budget and Collective Bargaining Agreements, as applicable
- Provides leadership to the Nursing Leadership Team and Charge Nurses in all human resource functions, including management/employee conflict resolution
- Ensures the provision of mandatory annual educational in-services for all nursing department employees and identifies nursing department learning needs.
- Manages and monitors all nursing envelope expenditures, monthly variances and works with the Administrator and Chief Executive Officer to identify corrective action
- Responsible for tracking High Intensity Needs / Lab costs and submission of the reconciliation to the MOLTC within required deadlines
- Ensures effective and efficient procurement and supply control procedures within the nursing department
- Provides leadership and direction to implement all aspects of the Quality Assurance and Risk management program as it pertains to nursing
- Establishes processes for the collection and analysis of clinical indicators within an established timeline on a monthly basis
- Liaises with the medical advisor and in collaboration with the Administrator and Chief Executive Officer evaluates his/her performance and ability to meet the requirements of the role
- Ensures quarterly summaries and MDS assessments are completed in PCC on time
- Liaise with the SSW/Placement Coordinator and/or Social Worker in reviewing and approving individuals appropriateness for placement
- Collaborates with administration, medical staff and other healthcare disciplines and services to meet the residents' healthcare needs, and report all issues affecting resident care i.e. staffing issues, increase in # of critical incidents, safety issues etc.
- In collaboration with the IPAC Lead, promote the safety of residents through the provision of Infection Prevention and Control measures and policies, through surveillance and outbreak management activities and the infection control committee, and the Risk Management programs
- Reviews and monitors nursing and personal care workers' contracts and quality of purchased supplies, equipment, and services and supply utilization.
- Performs any other duties as required and/or assigned
Qualifications:
- RN in good standing with a current certificate of competence from the College Nurses of Ontario.
- Current membership with the Gerontological Nursing Association (GNA) and the Registered Nurses Association of Ontario (RNAO) is an asset.
- Post - RN education, certification or degree in nursing administration and management, preference is BScN and/or business degree in health management, particularly long-term care, and further education in gerontology, geriatrics or rehabilitation.
- Demonstrates an understanding of current trends and concepts relative to professional Nursing.
- Exceptional oral and written communication skills including knowledge of computer-based documentation in the electronic health record
- Disciplinary-free record.
- Work within the scope and practice of applicable Standards of Practice as outlined by the College of Nurses of Ontario
- Strong understanding of the FLTCA’2021, MoLTC Compliance Requirements
- Demonstrated commitment to consistent attendance and punctuality.
- Possess a strong focus on customer service and resident-focused care
- Strong communication skills in English (written and oral) to interpret legislation, prepare reports, assist in the development of policies, and effectively convey information to resident care staff. Ability to speak Polish would be a definite asset for resident and family engagement.
- 3 years relevant management / supervisory experience in a long-term care facility including infection control, head nurse in chronic care, or senior management in a health community agency.
- Proficient in MS Office, Excel, PowerPoint, Outlook, and PCC electronic health record
- Ability to work in a fast-paced environment
- Ability to exercise good interpersonal and conflict management skills.
- Extensive knowledge of Infection Control Practices and RAI-MDS
- Ability to work flexible hours dependent upon demands i.e./compliance review times
- Ability to work weekends as required and share a rotational weekend on-duty schedule
- Knowledge in working with the electronic health record – Point Click Care
