Job summary

Empower Simcoe is seeking a Contract Full-Time Direct Care Staff—Awake Overnight to support people in a residential home in Barrie, Ontario. The six-month contract includes the opportunity to contribute to personal outcomes with overnight care and documentation duties.

Minimum requirements include a Grade 12 diploma and PSW certificate; driver’s license and reliable transportation are required. Starting wage is $25.66 per hour, with 11 paid statutory holidays and an Employee Assistance Program

Qualifications

• Minimum education: Grade 12 plus PSW certificate; other credentials may be considered.
• Experience in a community residential setting with people with disabilities is preferred.
• Familiarity with computer operations and software applications.
• Valid driver's license and reliable transportation to be used for work.

Responsibilities

• Provide support to residents using a Personal Outcome Measures lens.
• Assist residents to develop and realize individualized goals.
• Coordinate with Supports and Services Supervisor and Senior Support Worker on planning.
• Complete documentation accurately and timely as required by the supervisor.
• Support residents in achieving personal outcomes and provide personal care when needed.
• Monitor residents' well-being and report changes toOn-site coordinator; update records.
• Monitor behaviours, medication usage and health indicators as required and document in the system.

We are presently accepting resumes for a Contract Full-Time Direct Care Staff- Awake Overnight to provide supportive services during awake overnight shift to meet the needs of the people in accordance with the policies and procedures of Empower Simcoe and legislative requirements. This contract will be for a six-month period.

Compensation Package

• Work up to 72 hours biweekly;


• Minimum starting rate of pay is $25.66 per hour;


• 11 paid statutory holidays;

Minimum Requirements For This Position Include

• Proven successful completion of grade 12 high school diploma and a PSW Certificate. Other post-secondary academic credentials maybe considered for equivalency as determined by the employer.


• Previous recent work experience in a community residential setting providing support for individuals with disabilities is preferred and a definite asset.


• Knowledge and familiarity with computer operations and software applications.


• Current valid driver's licence and own reliable transportation to be used in the course of employment.

Overview Of Duties (including, But Not Limited To)

• Using a Personal Outcome Measures lens, act as the support person for people residing in the home


• Supporting people to develop and realize individualized goals through the Individual Support


• Planning process in accordance with the desire of the person and in conjunction with the Supports and Services Supervisor and the Senior Support Worker


• Completing all required documentation in a timely and effective manner, as outlined in Documentation Due reports and as requested by the Supports and Services Supervisor


• Actively participating and supporting the person to work toward achieving their personal outcomes


• Providing personal care when needed and visual checks throughout the night to all people supported in the home


• Maintaining vigilance over the physical and mental well-being of individuals, promptly reporting any changes in condition to the On-site coordinator, while accurately and diligently updating individual records as necessary


• Monitoring person's behaviour, psychoactive medication usage, possible blood sugar levels and other tracking as needed and provide feedback and recording to the team and on the Electronic Medication Administration System.
