Job summary

Empower Simcoe is seeking a Contract Full-Time Direct Care Staff for Awake Overnight to support people in our residential home in Barrie, ON. The six-month contract includes night-time care, documentation, and coordination with a supervisor.

Starting wage is 25.66 CAD per hour with 11 paid holidays and an employee assistance program for you and your family. PSW certification and a Grade 12 diploma are required.

Qualifications

• Minimum Grade 12 high school diploma and a PSW Certificate; other credentials may be considered.
• Experience in a community residential setting supporting individuals with disabilities is preferred.
• Knowledge of computer operations and software applications.

Responsibilities

• Act as the support person for people residing in the home using a Personal Outcome Measures lens.
• Help individuals develop and realize individualized goals through the Supports and Services Planning process.
• Complete required documentation in a timely and accurate manner as outlined in reports.
• Support people in achieving their personal outcomes and provide personal care during the night.
• Monitor well-being and report changes to the On-site coordinator; update records as needed.
• Record and communicate observations related to behavior and psychoactive medications as appropriate.

We are presently accepting resumes for a Contract Full-TimeDirect Care Staff- Awake Overnight to provide supportive services during awake overnight shift to meet the needs of the people in accordance with the policies and procedures of Empower Simcoe and legislative requirements. This contract will be for a six-month period.

Compensation package:

• Work up to 72 hours biweekly;
• Minimum starting rate of pay is $25.66 per hour;
• 11 paid statutory holidays;
• Employer paid confidential Employee Assistance Program for you and your family members;

Minimum requirements for this position include:

• Proven successful completion of grade 12 high school diploma and a PSW Certificate. Other post-secondary academic credentials maybe considered for equivalency as determined by the employer.
• Previous recent work experience in a community residential setting providing support for individuals with disabilities is preferred and a definite asset.
• Knowledge and familiarity with computer operations and software applications.
• Current valid driver's licence and own reliable transportation to be used in the course of employment.

Overview of duties (including, but not limited to):

• Using a Personal Outcome Measures lens, act as the support person for people residing in the home
• Supporting people to develop and realize individualized goals through the Individual Support
• Planning process in accordance with the desire of the person and in conjunction with the Supports and Services Supervisor and the Senior Support Worker
• Completing all required documentation in a timely and effective manner, as outlined in Documentation Due reports and as requested by the Supports and Services Supervisor
• Actively participating and supporting the person to work toward achieving their personal outcomes
• Providing personal care when needed and visual checks throughout the night to all people supported in the home
• Maintaining vigilance over the physical and mental well-being of individuals, promptly reporting any changes in condition to the On-site coordinator, while accurately and diligently updating individual records as necessary
• Monitoring person's behaviour, psychoactive medication usage, possible blood sugar levels and other tracking as needed and provide feedback and recording to the team and on the Electronic Medication Administration System.
