Job summary

About the position

Complete an assessment to evaluate clients health needs and diet; counsel clients on nutrition and healthy eating habits; develop meal plans; provide ongoing evaluation of the effects of meal plans and make changes as needed; develop and deliver education sessions on topics related to food and nutrition; deliver and facilitate nutrition programs; provide written reports to document client progress; other duties as assigned by the Home Care Manager.

Duties

• Working with the Home Care Manager & Diabetes Nurse Educator to develop and maintain nutrition programs;
• Complete assessments to determine client health needs and diet;
• Counsel clients on nutrition and healthy eating habits;
• Develop meal plans, taking both cost and client preferences into account;
• Promote healthy eating by speaking to groups about diet, nutrition and the relationship between good eating habits and preventing or managing specific diseases;
• Maintain liaison with the community & other related agencies, organizations, teachers, physicians and hospitals;
• Provide written reports to document client progress;
• Perform other related duties as assigned;
