Job summary

Unity Health Toronto is seeking a Registered Practical Nurse to contribute to inter-professional teams, provide autonomous nursing care to stable patients, and support patient education and discharge planning.

This role requires current Ontario RPN registration, recent hemodialysis experience preferred, BLS, and a commitment to evidence-based practice within a dynamic hospital setting.

Qualifications

  • Successful completion of a Practical Nursing Diploma Program.
  • Current registration as a Registered Practical Nurse with the College of Nurses of Ontario.
  • BLS Certification required.
  • Nephrology certificate preferred or enrolled in Nephrology 1 with completion within one year.
  • Experience with cannulation of fistulas/grafts preferred.

Responsibilities

  • Provides autonomous nursing care for patients who are stable with predictable outcomes.
  • Establishes a therapeutic relationship and coordinates patient care with the inter-professional team.
  • Documents observations, assessments and care provision in a timely manner.
  • Contributes to discharge and transition planning to facilitate continuity of care.
  • Educates patients and families using effective teaching strategies to address learning needs.
