Job summary

Your Support Services Network (YSSN) in Aurora, Ontario seeks a Developmental Services - Housing Case Manager on a full-time, contract basis through September 2027. The role requires on-site work in Aurora with a 35-hour week and a starting pay of CAD 32.13 per hour.

The incumbent will assess needs, coordinate service plans and connect clients to housing and community resources. Applicants should hold a Bachelor’s degree in social work or related field and have at least three years frontline

Qualifications

  • Minimum three years directly related frontline experience.
  • Experience working with individuals who have developmental disabilities.
  • Ability to assess needs and coordinate service plans.
  • Familiarity with the local service system and its providers.
  • Second language is considered an asset.

Responsibilities

  • Assess needs and develop coordinated service plans for individuals.
  • Provide referral support and skill development.
  • Support individuals to live more independently in their community and navigate services.
  • Connect clients to local community resources and agencies.
  • Maintain documentation and participate in team/staff meetings.

Developmental Services - Housing Case Manager

One Full-Time Contract to September 2027 (with the possibility of extension or becoming permanent)

  • Status: Full-Time,Contract
  • Starting Hourly Rate: $32.13/ Hour
  • Office Hours: Monday - Friday - 8:30 am - 4:30 pm,35 Hours per week
  • Location: Aurora, OntarioL4G 3S9
  • Office Expectations; This position requires:
    • A minimum of one in-office day per month to provide team coverage
    • Attendance at additional in-office meetings, training sessions, supervision meetings, and all-staff events as required
    • Applicants must reside within a reasonable daily commuting distance of the office and/or assigned service area and be able to attend the office as required for operational needs

Job Description

Primary functions include provision of assistance and support for adults who have a developmental disability and their families with a focus on transitioning to more independent living situations. This position will support individuals who would like to live more independently in their community, especially those who live with caregivers who are aging. The role will include support in navigating the service system and exploring all available community service options. The Housing Case Manager will be responsible for assessment of needs, development and coordination of service plans, referral support and skill development. They will also provide resources and support through community connections, the use of the DSO Housing Toolkit and by sharing information regarding community specific housing programs. They may also help individuals connect to local community resources and agencies including working with local municipal housing programs.

Position Requirements
  • Bachelor’s degree in social work or related discipline (i.e. Health Sciences)
  • Experience;
    • Minimum three years directly related front line experience
    • Experience working with individuals who have developmental disabilities is essential
    • Experience in assisting people to develop independent living skills and plan for more independent living is an asset
    • Language skills and an understanding of cultural diversity are considered assets
    • Familiarity with the local service system and its providers is required
  • Second language is considered an asset
  • An understanding of cultural diversity is considered an asset

* A valid G Drivers License, access to a reliable vehicle and adequate insurance coverage for business responsibilities, OR access to reliable transportation.
