Your Support Services Network (YSSN) in Aurora, Ontario seeks a Developmental Services - Housing Case Manager on a full-time, contract basis through September 2027. The role requires on-site work in Aurora with a 35-hour week and a starting pay of CAD 32.13 per hour.

One Full-Time Contract to September 2027 (with the possibility of extension or becoming permanent)
Primary functions include provision of assistance and support for adults who have a developmental disability and their families with a focus on transitioning to more independent living situations. This position will support individuals who would like to live more independently in their community, especially those who live with caregivers who are aging. The role will include support in navigating the service system and exploring all available community service options. The Housing Case Manager will be responsible for assessment of needs, development and coordination of service plans, referral support and skill development. They will also provide resources and support through community connections, the use of the DSO Housing Toolkit and by sharing information regarding community specific housing programs. They may also help individuals connect to local community resources and agencies including working with local municipal housing programs.
* The successful applicant will be required to provide a police clearance that meets agency standards
* A valid G Drivers License, access to a reliable vehicle and adequate insurance coverage for business responsibilities, OR access to reliable transportation.
