Your Support Services Network in Aurora, Ontario, is hiring a Mental Health Case Manager to provide comprehensive community-based case management for adults 16+ with serious mental illness or addictions.
You will work within a Recovery Model to identify needs and goals that improve quality of life, collaborate with service users and families, and participate in office-based and field activities as required.

One Full-Time Contract to February 2028 (with the possibility of extension or becoming permanent)
In this role, you will provide comprehensive and collaborative community based case management response to individuals age sixteen and over with serious mental illness and/or addictions within York Region. Utilizing a Recovery Model framework, the Mental Health Case Manager will assist service users to identify needs and goals that will enhance the quality of their lives.
Successful applicant will be required to provide a police clearance that meets agency standards
A valid G Drivers License, access to a reliable vehicle and adequate insurance coverage for business responsibilities, OR access to reliable transportation.
