Job summary

The Saskatchewan Health Authority in Watrous is seeking an Assessor Coordinator for Home Care to assess and coordinate client services and case management in the Watrous District Health Complex. Qualifications

• Degree in health or social science related field.
• Experience in palliative care considered an asset.
• A valid driver's license and a vehicle is required to carry out duties.
• Registered/eligible for registration with the appropriate licensing body.

• Refer clients to other appropriate agencies and long-term placement.
