Job summary

Autism Services Lanigan is seeking a Pay Band Assessor Coordinator with a Bachelor of Science in Nursing (BScN) or an acceptable nursing/social work combination to assess clients and coordinate home care services in the Lanigan area. The role reports to the Home Care Manager and supports long term care placement in rural communities.

Travel is required within the region; the position is full-time and permanent, with days shifts and a 56-hour rotation across 12 weeks.

Qualifications

• Bachelor of Science in Nursing or an acceptable equivalent.
• Additional social work or nursing diplomas may be accepted.
• Experience in home care coordination is preferred.

• Job Description: Reporting to the Home Care Manager the Client Case Coordinator is responsible for the assessment of clients, coordination and evaluation of home care services and case management of region residents. The Client Case Coordinator will be responsible for long term care placement in rural under the Home Care program. The Client Case Coordinator will ensure that the delivery of home care and case management services is consistent with the policies, vision, mission and philosophy of the Saskatchewan Health Authority.
