Health Sciences North seeks a Coordinator to lead the Regional Outpatient Geriatric Medicine Service, organizing day-to-day operations and directing inter-disciplinary teams to deliver high-quality patient care.

You will manage staff, budgets, policies, and reports, ensuring safety and compliance while coordinating training and resource planning across northeastern Ontario.

Qualificações

  • Bachelor’s degree in a health-related field; Master’s degree preferred.
  • Current registration with a regulatory college.
  • MOHLW Health and Safety Awareness training.
  • Minimum five years as a Health Professional in a health care environment.
  • Minimum two years in a leadership/supervisory role.
  • Minimum five years in Specialized Geriatric Services.

KEY FUNCTION

Provide leadership and direction through planning and organizing of the day-to-day operations of the Regional Outpatient Geriatric Medicine Service. Promote the delivery of optimal care/service to patients throughout the North East, through the management of human and material resources.

REPORTING

Under the general direction of the Clinical Manager, Regional Outpatient Geriatric Medicine Service.

DUTIES
  • 1. Design, promote, and facilitate the use of systems to ensure that the provision of patient care is consistent with program and professional standards.
  • 2. Identify and respond to problems and deficiencies in the delivery of optimal patient care and facilitate an inter-disciplinary approach to the formulation of solutions.
  • 3. Support work schedules and staff assignments to ensure deliverables are met.
  • 4. Coordinate and participate in clinical activities that require an interdisciplinary team.
  • 5. Ensure program delivery is consistent with evidence-based and best practices.
  • 6. Prepare and ensure timely communication/reports to all internal and external stakeholders (e.g. database management, statistical analysis and reporting to funding bodies).
  • 7. Act as a resource to staff in client care matters and ensure patient/client needs are met in a safe and timely manner.
  • 8. Act as liaison for external inquiries.
  • 9. Support human resources which include: recruitment, selection and training.
  • 10. Develop, review and revise policies, procedures, work standards, and guidelines to ensure excellent service delivery and adherence to administrative policies and procedures.
  • 11. Assist with budget development and regular monitoring of financial outcomes.
  • 12. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
  • 13. Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.
  • 14. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  • 15. Educate and promote health, safety and wellness in the work place.
  • 16. Represent the department or program on various committees and in meetings as required.
  • 17. Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING
  • 1. Minimum of a four (4) year Bachelor’s Degree with a specialization in a health-related field, from an accredited university.
  • 2. A Master’s Degree from an accredited university is preferred.
  • 3. Current Certificate of Registration in good standing with an applicable regulatory college is required.
  • 4. Ministry of Labour “Worker Health and Safety Awareness” training certificate is required.
EXPERIENCE
  • 1. Minimum of five (5) years’ experience working as a Health Professional within a health care environment.
  • 2. Minimum of two (2) years’ working in a leadership/supervisory role, preferably in a health care environment.
  • 3. Minimum of five (5) years’ experience working within Specialized Geriatric Services.
KNOWLEDGE/SKILLS/ABILITIES
  • 1. Demonstrated superior conflict management and resolution skills.
  • 2. Proven ability to develop, monitor, administer and report on budgets.
  • 3. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
  • 4. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  • 5. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  • 6. Demonstrated superior interpersonal and communication skills, both written and verbal.
  • 7. Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.
  • 8. Demonstrated discretion and maturity when handling confidential information.
  • 9. Demonstrated commitment to the safety of co-workers and patients.
