Department: Nipissing Specialized Geriatric Services
As part of the Nipissing Specialized Geriatric Services, the RN with the Nipissing Specialized Geriatric Clinic works collaboratively alongside Behavioural Supports Ontario, seniors' Mental Health - Regional Consultation Service, and key partners to promote physical wellness and independence, prevent hospitalization and optimize overall quality of life for the older adult and their care partner(s). The successful RN/Geriatric Assessor will offer outreach services including Comprehensive Geriatric Assessment for older adults in the community and within partnering Primary Care clinics. The RN/Geriatric Assessor will work within a compassionate person-focused philosophy aligned with best practice principles and frameworks and in accordance with respective standards of clinical practice. Reporting to the Manager and Director of Nipissing Specialized Geriatric Services, the successful incumbent will demonstrate a commitment to NBRHC's organizational values.
QUALIFICATIONS
Certification/Experience/Knowledge
- Current Certificate of Registration in good standing with the College of Nurses of Ontario required
- Minimum of three (3) years of clinical experience in the care of older adults required
- Current B.L.S. Certification (within 12 months) required
- Demonstrates extensive knowledge and applied clinical skills working with older adults with various medical complexities, geriatric syndromes, neurocognitive impairments, and mental health conditions required
- Applied knowledge of health assessment tools for the older adult population (e.g. Comprehensive Geriatric Assessment (CGA), Mini Mental Status Examination (MMSE), Montreal Cognitive Assessment (MoCA), Clock drawing test, Geriatric Depression Scale, Cornell Scale for Depression in Dementia, Barthel Index, Confusion Assessment Method (CAM) required
- Certification in administration of the Montreal Cognitive Assessment (MoCA) required
- Demonstrated skills in management of geriatric syndromes, including delirium, polypharmacy, falls and functional decline, acute/sub-acute cognitive changes, bowel/bladder, and nutrition/hydration problems
- Demonstrated skills conducting comprehensive assessments, analyzing, developing and administering treatment plans with older adults
- Demonstrated knowledge of medication management including medication reconciliation, promotion of medication adherence, identification & monitoring of side effects and awareness of polypharmacy
- Demonstrated knowledge of Specialized Geriatric Services across the continuum including primary care and community support services (i.e. Alzheimer Society, Ontario Health atHome, etc.)
- Demonstrated ability to exercise professional judgement, clinical reasoning, and critical thinking with the ability to apply ethical frameworks and choose the optimal assessment and treatment intervention
- Demonstrated ability to interact with physicians and other healthcare providers in a collaborative, professional, intelligent, and effective manner
- Demonstrated clinical competence and inter-collaborative skills to promote, facilitate and integrate best practice principles in the care of older adults required
- Certification in the following fundamental specialized geriatric health core competencies: Provincial Geriatrics Leadership Ontario (PGLO)Common Orientation, Gentle Persuasive Approach (GPA), Physical, Intellectual, Emotional, Capabilities, Environment, Social (PIECES)required
- Experience and demonstrated commitment to quality improvement and change initiatives required
- Indigenous Cultural Safety Training certificate preferred
- Trauma-Informed Core competency training preferred
- Knowledge of consent and capacity matters and legislation applicable to this older adult population (e.g. Public Guardian Trustee, Substitute Decision Maker Act, Health Care Consent Act, Mental Health Act, Long-term Care Act, Personal Health Information Protection Act (PHIPA)) required

- Excellent written and verbal communication skills to effectively document and convey essential clinical and systems information required
- Demonstrated cultural awareness and competence in working with older adults and care partners from diverse backgrounds and across various sectors required
- Proven interpersonal and critical thinking skills to establish and maintain positive working relationships with patients, care partners, team members and partners required
- Proven organizational and time management skills, along with the ability work in a self-directed manner required
- Demonstrated good attendance and self-awareness with regard to work life balance required
- Proficiencies in navigating relevant technological applications (e.g. Microsoft Office, Meditech)
- Expanse, Ontario Telemedicine Network, other video-conferencing platforms {Teams}, etc.) required
- Must have a valid driver's license, willingness to travel on a regular basis and access to full time use of reliable vehicle is required
- Bilingualism (Advanced level English/French) is required. All unilingual candidates are encouraged to apply and may be considered if the identified French requirement has been met or there are no successful bilingual candidates. A formal French test will be conducted to evaluate the proficiency level.
