Job summary

North Bay Regional Health Centre is seeking a Patient Flow Coordinator to support the Manager of Patient Flow in developing and monitoring the hospital-to-home program with NWOT and partners. You will help shape referrals, rounds, and discharge processes to ease transitions.

The role requires strong leadership, policy development, and collaboration across disciplines, with a focus on improving patient outcomes and care coordination in a busy hospital setting.

Qualifications

• Bachelor’s or Master’s degree in a health-related field.
• Registration and good standing with a regulatory body (Ontario).
• Memberships: N S W/SW, OT, PT, Audiologists/Speech-Language Pathologists, etc.
• Minimum 3–5 years of Medical/Surgical experience.
• Strong leadership and quality-improvement focus.
• Ability to work independently within a multidisciplinary team.

Responsibilities

• Review referrals and coordinate Hospital to Home steps with teams.

Department: Patient Flow Coordinator

As a Patient Flow Coordinator, you will support the Manager of Patient Flow with essential duties and the development, implementation, and monitoring of the hospital-to-home program in collaboration with Northeastern Ontario Together (NWOT), service providers and other relevant partners.

Essential Duties of the Patient Flow Coordinator include, but are not limited to:

• Assist the Manager of Patient Flow with the schedule of Patient Flow staff, standard work documents, departmental policies, program development and quality improvements.


• For Hospital to Home (H2H), work closely with the interdisciplinary team in reviewing referrals, sending referrals, attending rounds and ensuring barriers are reduced for patients discharges.


• Coordinate and monitor care plans working in collaboration with patients, families and the service provider


• Monitor and report on key patient indicators in collaboration with Service Provider Organizations (SPOs)

QUALIFICATIONS

Certification/Experience/Knowledge:

• Bachelor’s or Master’s degree in a health-related field


• Certificate of registration and in good standing with one of the applicable regulatory bodies


• College of Nurses of Ontario


• College of Physiotherapists of Ontario


• College of Occupational Therapists of Ontario


• Ontario College of Social Workers and Social Service Workers


• College of Audiologists and Speech Language Pathologists of Ontario


• Minimum of 3-5 years of Medical/Surgical experience required


• Demonstrated leadership experience and management abilities


• Must be able to function both independently and as an active member of the multidisciplinary team in addressing complex patient and family needs is required.


• Must be involved in continuing education on an annual basis and demonstrate commitment to quality improvement required


• Computer application experience required


• Experience in policy and procedure development is an asset


• Knowledge of the OPSEU and ONA collective agreements

Skills/Abilities:

• 2+years of recent experience in health or a related field


• Knowledge of the health care delivery system and community resources


• Demonstrated leadership, change management, problem solving, critical thinking, conflict resolution, team building and communication skills


• Effective time management, prioritization and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environment


• Established ability to accurately complete required documentation, reports and forms


• Proficient in Microsoft Word, PowerPoint, and Excel required and HIS expanse


• Good attendance and work record required.


• Experience working with diverse patient groups, e.g., multicultural, palliative, geriatrics, an asset.


• Bilingualism (Advanced level English/French) is an asset.
