Job summary

Alberta Health Services seeks a Licensed Practical Nurse to assess, plan, monitor, and evaluate clients in the community, coordinating care with families in clients' homes or alternate settings, under the Home Care program.

You will work with the interprofessional team to provide safe, high-quality patient and family centred care aligned with standards and policies; role includes collaboration with the RN Case Manager and ongoing professional development.

Qualifications

• Completion of an accredited practical nurse education program.
• Active or eligible for registration and a practice permit as a Licensed Practical Nurse with CLHA.
• Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Responsibilities

• Assess, plan, monitor and evaluate clients in home care.
• Coordinate and provide direct care in client homes or alternate settings.
• Collaborate with the Registered Nurse Case Manager and families to ensure safe, high-quality care.
