Job summary

Wesley in Halton Region is hiring a Community Wellness Worker to support residents with hoarding concerns, improving wellbeing and social functioning. The role is mobile within Halton, helping tenants access services and reduce risk of eviction.

The position is full-time, six-month contract ending March 31, 2027, with a 40-hour work week and a pay rate of $27.32 per hour. Prior CBT experience and trauma-informed practice are assets.

Qualifications

• Post-secondary education from a recognized university in the field of gerontology, Social Work, or other human services discipline.
• A valid Driver’s License and a reliable personal vehicle are required.
• Non-violent crisis intervention and 1st aid/CPR training are required.
• 2-5 years of relevant, practical experience with the target population.
• Ability to register with recognized college registration such as OCSWSSW or CPRO.
• CBT certified and/or extensive knowledge and experience in CBT program delivery would be an asset.
• Strong understanding or experience of trauma-informed care and hoarding issues.

Responsibilities

• Assess, triage, and provide mental health and wellness services to clients identified and accepted into the program.
• Facilitate supportive discussions around stressors, trauma, and related issues.
• Engage clients in the weekly CBT structured program for hoarding.
• Collaborate with the Manager to develop, implement and monitor case plans.
• Complete administrative tasks and maintain recording/tracking systems; respond to queries.
• Work with professionals from other organizations to support clients.
• Collaborate with the Halton Hoarding Coalition to implement best practices.
• Adhere to Wesley policies and participate in meetings and training.
• Attend staff meetings and other duties as assigned.

Are you the one we’re looking for?

We are seeking a dynamic, highly motivated Community Wellness Worker who demonstrates our values of compassion, accountability, responsiveness, and empowerment; to our clients, families, and each other.

The Community Wellness Worker is a mobile position within the Halton Region providing supports to residents of Halton who have been assessed for hoarding concerns, to increase each individual's well-being and mental health, to resolve or mitigate troublesome hoarding behaviors, beliefs, compulsions, thoughts, or emotions, and to improve relationships and social functioning.

This position supports clients, reduces evictions and tenants’ need to access emergency shelter, and works with tenants to problem-solve when tenancies are at risk. The Community Wellness Worker will provide direct services to tenants and also help tenants connect to needed services in the community.

This is a full-time, temporary unionized position with a six-month contract ending March 31, 2027, with the possibility of an extension. The position offers 40 hours per week at a rate of $27.32 per hour.

Key Responsibilities:

1. Assess, triage, and provide mental health & wellness services to clients identified and accepted into the program.


2. Facilitate opportunities to engage individuals in a supportive manner that will support discussion around stressors, trauma, and other issues that may impede healthy functioning.


3. Work with clients to complete the weekly CBT structured program for hoarding.


4. Work collaboratively with the Manager to develop, implement and monitor case plans.


5. Complete administrative tasks, maintains effective recording/tracking systems, and responds to queries.


6. Work in partnership with professionals from other organizations that support clients such as social care, health, police, education, youth-serving agencies and early childhood services.


7. Collaborate with the Halton Hoarding Coalition to implement best practices.


8. Adhere to the policies and procedures of Wesley.


9. Attend staff meetings and training sessions as required.


10. Other duties as assigned.

What you will bring:

1. Post-secondary education from a recognized university in the field of gerontology, Social Work, or other discipline related to human services.


2. A valid Driver’s License and a reliable personal vehicle are required.

4. 2-5 years of relevant, practical experience with the target population.


5. Ability to register with recognized college registration such as OCSWSSW or CPRO.


6. Psychotherapy experience working with hoarding issues, dealing with trauma, emotional and spiritual issues would be a valuable asset.


7. Excellent problem-solving, organization, and coordination skills, and ability to work effectively independently by implementing self-accountability techniques and a strong understanding of support services in the community.


8. CBT certified and/ or extensive knowledge and experience in CBT program delivery would be an asset.


9. Strong understanding or experience of working with victims of trauma, supporting through a trauma-informed lens.


10. A strong foundation and understanding of hoarding disorder.


11. Empathetic and non-judgmental approach in supports.


12. Familiarity with community resources and understanding of collaborative approach with other community agencies.


13. Proven ability to demonstrate Wesley’s mission, vision, and core values.


14. Solid communication skills, both written and verbal in English. Second language relevant to the client population is an asset.
