Job summary

VON Canada is a not-for-profit organization delivering home and community care with a patient-centered focus. The role supports children to seniors in clients' homes, clinics, or virtual settings, collaborating with an interdisciplinary team to plan and provide holistic care.

The nurse will perform assessments, administer medications, manage chronic diseases, and contribute to palliative and rehabilitative services, with driving required in some cases.

Qualifications

• Must be licensed as a Registered Practical Nurse in the province of practice.
• Community health experience is an asset.
• Must enjoy driving, have a valid driver's license and access to a reliable vehicle.
• Satisfactory Criminal Records Check and related screenings may be required.
• Must be able to wear Personal Protective Equipment (PPE).

• Client assessments.
• Provide primary care in collaboration with other healthcare professionals.

• Chronic Disease Management, e.g. diabetes care and education.
• Specimen collection.
• Rehabilitative programs.
