West Suffolk NHS Foundation Trust is seeking a Community Nurse for the Newmarket Integrated Neighbourhood Team. The role involves home visits, assessing patients and delivering individual care plans, working closely with the Virtual Ward and Early Intervention Team.

Newly qualified and experienced nurses are welcome; RN/NMC registration is required or awaiting. The post includes leg ulcer and doppler clinics, with a strong emphasis on teamwork, mentoring, and high-quality community care.

Qualifications

• RN qualification and NMC registration or awaiting registration.
• Strong communication and interpersonal skills with patients and carers.
• IT literacy and accurate patient record entry including clinical observation and reporting.

Responsabilités

• Assess, plan and deliver nursing care to patients in their homes during visits.
• Develop and implement individualised care plans for each patient.
• Deliver care in specialist clinics (e.g., leg ulcer and doppler).
• Collaborate with the Virtual Ward and Early Intervention Team to support early discharge.
• Work with health professionals and agencies to provide integrated services.
• Maintain safety and handle equipment within budget and policies.
• Provide mentorship to students and supervise junior staff.
