Job summary

Cambridgeshire and Peterborough NHS Foundation Trust is seeking a motivated Care and Wellbeing Navigator to support a developing Tier 3 Weight Management Service. You will act as a central point for referrals, patient engagement and service administration, ensuring a seamless patient journey and strong collaboration with clinicians and partner organisations.

The role involves data management, meeting coordination and performance monitoring, with flexible working and opportunities for development

Qualifications

• Experience in an administrative, coordination or patient-facing role.
• Able to manage competing priorities and stay organised.
• Confident and professional communication with diverse stakeholders.
• Strong IT skills including Microsoft Office and database systems.
• Ability to work independently while contributing to a team.
• Compassionate, person-centred approach to care.
• Interest in health improvement and behaviour change.
