Job summary

Qualifications

• Post-secondary education in social services or equivalent.
• Experience in social services, mental health, or equivalent.
• Valid driver’s license.
• Clear criminal record check.

Responsibilities

• Build meaningful relationships with clients and families.
• Coach life skills and support independence.
• Assist to design and implement person-centred support plans.
• Provide personal care as needed.
• Assist with meal preparation and diet planning.
• Medication administration and documentation.
• Transport individuals to appointments and activities.
• Maintain a safe work environment and report safety concerns.
• Coordinate with Chestnut House Program Manager and stakeholders.
