Job summary

Maple Ridge/Pitt Meadows Community Services is seeking a Community Support Worker to provide one-on-one support to adults living with mental health challenges within the community.

The role focuses on promoting independence, safety, and access to services, with a 35-hour work week, regular weekday schedule, and wage starting at CAD 29.22/hour. A two-year diploma in social services and relevant experience are required, along with a valid BC driver’s license and access to a vehicle.

Qualifications

  • Criminal record check is required.
  • Current First Aid and CPR certificate.
  • Valid BC Class 5 driver's license with access to a vehicle.
  • Clear driver’s abstract annually.
  • Union membership required.

Responsibilities

  • Facilitate life skills training including self-care, home management, community management and social leisure skills.
  • Assist clients in gaining knowledge of available services and how to access them; advocate for self.
  • Assist clients in acquiring appropriate living accommodations.
  • Provide assistance navigating environments such as home, grocery stores, banks, etc.
  • Provide updates to the client’s Primary Therapist per the Individual Services Plan.
  • Advocate/liaise on behalf of the client to address concerns with case manager.
  • Support clients by referring issues to the client’s Primary Therapist (mental health) such as medication, suicidal ideations, family concerns, changes in functioning.
  • Maintain transparency with the client before connecting with the case manager.

Job description

Community Support Worker

Our Community LivingSupport program provides one on one support to individuals living with mentalhealth challenges. We focus on the direct needs of individuals, supporting them in realizing their full potential, achieving independence, and pursuing their hopes and dreams.

In this full time(35 hours per week) position, our CommunitySupport Worker will be responsible for supporting clients with severe and persistent mental health issues to live independently within the community. The majority of the work is done with clients in their home and/or within the community.

ACCOUNTABILITIES
  • Facilitating life skills training thatincludes teaching of self-care, home management, community management andsocial leisure skills.
  • Assisting clients in gaining necessaryknowledge of available services within the community, how to access servicesindependently, and how to advocate for self.
  • Assist client in gaining the skills necessaryto successfully acquire appropriate living accommodations.
  • Provide assistance navigating a variety ofenvironments such as home, grocery stores, banks, etc.
  • Provide updates to the clients’ PrimaryTherapist to fulfill case conferencing expectations as per the IndividualServices Plan Agreement.
  • Advocating/liaising on behalf of the client todeal with any concerns/issues at hand, as required.
  • Supporting clients by referring pertinentissues to the client’s Primary Therapist at Mental Health, (e.g. medication,suicidal ideations, family concerns, changes in client’s functioning level).
  • Transparencywith the client (if possible) is a vital part of the conversation prior toconnecting with the case manager.
QUALIFICATIONS
Education, Training and Experience:
  • Minimum of a two-year diploma or degree in asocial services related field.
  • Minimum of one year of relevant experienceworking with adults living with mental health issues.
  • Knowledge of the principles of psycho-socialrehabilitation.
Additional Requirements:
  • Criminal Record Check is required.
  • Current First Aid and CPR Certificate.
  • Valid and unrestricted Class 5 BC Driver’sLicense with access to a safe and reliable vehicle.
  • Clear Driver’s Abstract (provided annually).
  • Position requires Union Membership.

Shift/Schedule/Hours per week: Regular FT; 35 hours perweek, Monday to Friday 8:30 am – 4:30 pm

Expected Start Date: ASAP

Wage/Salary: Starting at $29.22/hour

Grid Level: 11
