Qualifications

• 3 - 5 years of job-related experience or an equivalent combination of education and experience.
• Registration with the College of Nurses of Ontario (CNO) is required.
• Experience in a community healthcare environment considered an asset.

Responsibilities

• Provide effective coordination and supervision of Community Health Services.
• Develop and maintain effective Care Plans.
• Mentor Community Support Workers.
