Job summary

Winnipeg Regional Health Authority (WRHA) is seeking a Systems Navigation Support Worker as part of the My Health Team. The role focuses on enhanced primary care service integration, helping clients navigate the health system and connect with community resources.

You will work with a multidisciplinary team in clinics, homes, and community settings, providing outreach, information, and support to clients who may face health inequities.

Qualifications

• Two years of related experience with people with mental health issues or other human service.
• Experience with populations facing health inequities and determinants of health.
• Experience/education with de-escalation techniques.
• Life skill teaching and support to individuals with mental health issues or disabilities.
• Experience in developing, planning and coordinating services.
• Requires a valid driver's license and a vehicle.

Responsibilities

• Assist in carrying out individual service plans under the primary care provider.
• Support rehabilitative and supportive functions including skill building and development of supports.
• Link clients to formal and informal support networks and community resources.
• Provide information and consultation to families about skill building and supports.
• Engage in outreach to homebound clients and work across clinic and community settings.
