JOBSUMMARY
Reporting tothe Collaborative Practice Team Leader, the Collaborative Practice Coordinatorworks collaboratively in a neutral role with the social worker, immediate andextended family (kinship), community members and other supports identified byfamily to develop a plan of care to address child safety concerns. TheCoordinator works collaboratively with Program Social Worker to ensure that thefamily understands the issues identified in the structured decision makingtools and also explains how the process is intended to deal with these issues.
PURPOSEOF POSITION
The Coordinatorfacilitates and helps to implement the co-operative family directedintervention planning process that is available to families whose children areidentified by the Director to be in need of safety. It provides anon-adversarial route for developing risk reduction plans but is not intendedto replace the court process. The parents will have the option of using thismethod or choosing alternative methods of dispute resolution including court.The participation of the immediate and extended families is a vital part ofthis initiative. This process allows the families to be directly involved withthe creation of the plan or if necessary, in a supervisory role.
Mostintegral to this process is the Coordinator working to ensure the family issupported by allowing the family to identify their own strengths, supports andto implement the meeting in such a way that honors and reflects their culturalidentity, practices and protocols.
KEY DUTIES ANDRESPONSIBILITIES:
Planning andFacilitation by:
- Interviewingthe client and family.
- Assessingthe problem.
- Providinginformation about the organization to the client and family.
- Contactingthe clients and families to assess the willingness to participate in programs.
- FamilyGroup Conferences are family-driven processes, that involves a private familytime component and is not time limited.
- CollaborativePlanning Conferences are scheduled 90-minute meetings utilized to addressimmediate safety planning for families.
- Thecoordinator will also facilitate other meetings at request of the Team Leadersuch as; Integrated Case Management meetings and other informal family planningmeetings.
Provides and prepares all professionalparticipants to ensure clarity of roles by:
- Providinginformation and referrals to other community service providers, resources andprofessionals as deemed necessary.
- ContactingSocial Workers assigned to the case and ensuring that all relevant informationconcerning risk and safety concerns involving client and family are met.
- Workingwith a professional team and liaising with colleagues and other professionals.
- Meetingwith families in their homes or communities to identify conference participantsand to clarify confidentiality and potential safety issues.
Integrated CaseManagement:
- Ensuringthat the family has an advocate or support person to attend the meetings andany important conferences as required
- Ensuringthat the views of the child and/or children are always heard
- Developingan analysis and plan of action around issues of resistance or reluctance
- Explainingthe goals and effectiveness of their participation in the process
- Establishingan atmosphere of safety and respect that supports the strengths and integrityof the family
- Involvingthe family members to establish guidelines about how they would like thediscussions to happen
Maintains case filesby:
- Preparingand submitting meeting notes in a timely manner.
- Ensuringthat any changes made to the Family Plan are well documented
- Preparingand providing information on assessments, progress reports, completing caseloadstatistics for client files and providing this information to the Supervisor
- Ensuringthe FGC data system is accurately updated with the required information
QUALIFICATIONS
Education and Experience:
1.Preferred Credentials:
- Bachelor’s Degree or higher in Social Work or in Child& Youth Care OR Master's Degree in Educational CounsellingPsychology/Master of Arts in Counselling Psychology with completion of apracticum in family and child welfareOR
- A candidate may be considered equivalent when they areregistered and a member in good standing with the BC College of Social Workersor has a letter from the College confirming that their application forregistration has been approved.
- C6 Delegation (to be provided).
2.Minimum Credentials:
- Bachelor’s Degree in a Human Services fieldORBachelor’s Degree witha Major/Honors in a Human Services fieldORMaster’s level graduate Certificate/Diploma in a Human Services field AFTER the completion of an unrelated bachelor’s degree.
- PLUS, a minimum of one year of relatedORjob-specificwork experience.
- C6 Delegation (to be provided).
- Human Services fields include:Psychology, Sociology, Criminology, Anthropology, Early Childhood Education,Indigenous Studies, Education, Theology or Nursing.
Job Skills and Abilities:

- Knowledge of First Nations living off reserve inurban areas and the systemic barrier challenges experienced by them
