Job summary

UHN Medical Solutions, a division of Altum Health, seeks a Clinical Coordinator to lead complex patient care coordination across multiple specialties at Toronto Western Hospital. The role reports to the Senior Manager and supports a fast-growing international patient program within a collaborative startup-like environment.

Requires a regulated health profession, 3+ years of clinical coordination, and proficiency with MS Office tools.

Qualifications

• At least 3 years of clinical practice or clinical coordination experience.
• Regulated healthcare professional (RHPA)
• Experience incorporating equity, diversity, inclusion, and accessibility principles into daily practice.
• Members in good standing, or eligible for RHPA registration.
• Knowledge of privacy, confidentiality, and consent requirements (PHIPA / PIPEDA).

Responsibilities

• Provide clinical leadership in coordinating complex patient cases across multiple services.
• Assess patient needs using clinical information and professional judgment.
• Act as the central liaison among patients, families, physicians, and providers to ensure integrated care.
• Collaborate with physicians and care teams to support reviews and care planning.
• Facilitate access to consultations, diagnostics, procedures, and follow-up services.
• Develop relationships with providers, insurers, and funding bodies to support continuity of care.
• Navigate funding and authorization processes, coordinating documentation with payors.
• Contribute to quality improvement and program development initiatives.
• Participate in cross-functional initiatives promoting innovation and collaboration.
