Job summary
CareRx is seeking a Clinical Consultant Pharmacist to join our Calgary team. You will maintain continuity of care across long term care, supportive living and congregate settings by collaborating with onsite physicians, nursing staff and other health providers.
The role involves clinical pharmacy activities in line with provincial standards, travel to LTC homes, education for residents and staff, documentation and audits, and participation in clinical program development.
Qualifications
- Licensed with the Alberta College of Pharmacy (ACP) in good standing.
- Foster partnerships with healthcare team.
- Knowledge of provincial LTC regulations.
- Strong customer service and communication skills.
- Ability to work independently with minimal supervision.
Responsibilities
- Act as liaison between LTC homes and CareRx Pharmacy to ensure resident care.
- Travel to LTC homes and retirement communities.
- Complete medication reviews for residents in serviced communities.
- Complete required documentation and regulatory compliance.
- Provide education to Residents and staff on health topics (e.g., MedsCheck).
- Attend and participate in clinical team meetings.
- Support development of clinical pharmacy services.
