Job summary

CareRx is seeking a Clinical Consultant Pharmacist to join our Kelowna team, providing continuity of care for residents through collaboration with onsite physicians, nursing staff, and other health providers at long-term care and retirement communities.

You will perform clinical pharmacy activities in alignment with Continuing Care Health Service Standards, site policies, and ministerial orders, with a defined 5-day work week and travel to communities in the Kelowna area.

Qualifications

• Licensed in good standing with Ontario College of Pharmacists, or College of Pharmacists of British Columbia (RPhT), or Alberta College of Pharmacy (ACP).
• Proven ability to foster strong partnerships with other members of the healthcare team.
• Excellent organization, planning, and time management skills.
• Knowledge of Kroll Windows application and Microsoft Office programs.
• Complete understanding of provincial regulations governing LTC homes and Retirement Communities.
• Must be familiar with the provincial standards of practice established for LTC pharmacies.
• Excellent customer service and interpersonal skills, oral and written communication skills and the ability to interact with persons of varying levels of professional expertise.
• Self-directed requiring minimal supervision and able to work independently.

Responsibilities

• Act as liaison between long term care homes and retirement communities and CareRx Pharmacy to ensure optimal resident care and clinical support.
• Travel to LTC homes and retirement communities to perform responsibilities.
• Build and maintain relationships within the community to develop strong partnerships and collaboration.
• Complete medication reviews in retirement communities and LTC homes serviced by CareRx Pharmacy.
• Complete required documentation and billing in line with MOHLTC, ACP, and other standards and regulations.
• Provide education to residents and staff on health topics and participate in community events as required.

Role Accountabilities:

• Act as aliaisonbetweenlong term care homesand retirement communitiesand CareRx Pharmacyto ensure optimal Resident care, Pharmacy Services, and Clinical support
• Travel to LTC homes and retirement communities
• Build and maintain relationships within the community to ensure ongoing collaboration and communication between home/communityleadership teams and pharmacy teams to develop/maintain strong partnerships
• Complete medication reviews in currently serviced retirementcommunities and long-term care homesserviced by CareRx Pharmacy
• Complete all required documentation and billing processes as required by MOHLTC, Alberta College of Pharmacy (ACP), and other federal and provincial standards and regulations
• Provide education to Residents and staff on various health-related topics including theMedsCheckprogram and attend community-based wellness fairsand other eventsas required
• Conducteducation sessions withinLong Term Care Homes and Retirement Communities withinscope of practice, as applicable (e.g. policy and procedure, web portal,eMAR, influenza protocol, etc.)
• Support CareRx Pharmacy policies and procedures within retirementcommunities and long-term care homes
• Completemedication audits on-site in LTC Homes and Retirement Communitiesas required
• Ensure regular on-site visits are made to all assigned clients
• Attend/participate in annual professional conferences and trade shows, when applicable
• Participate in the development and implementation of industry-leading clinical pharmacy service, when applicable
• Be an education, policy and pharmacy initiative resource
• Be available for special clinical team projects as required
• Complete/collaborate in the develop of medication audit follow-up improvement plans when applicable
• Works collaboratively with other team members to support applicable homes and communities
• Participate in pharmacy-related transition processes (e.gnew client transitions,eMARtransitions, etc.)
• Help with pharmacy operations/drug distribution when required
• Attend and actively participate in clinical team meetings

Compensation Range:$110,000-$120,000 annual salary pro-rated over a 5-day work week .Actual pay rate will be based on the individual candidate’s credentials and experience.

Location:Location will primarily be LTC homes and retirement communities in the Kelowna and Kootenay’s surrounding area such as Cranbrook, Kimberley.

Work Shifts:This is a full-time position working 5 days a week, with flexibility and opportunity to work-from-home and on-site from the care homes.
