Job summary

CareRx Corporation is seeking a Clinical Consultant Pharmacist to maintain continuity of care in long-term care and retirement communities. You will collaborate with onsite physicians, nursing staff, and health providers to deliver clinical pharmacy services in alignment with standards and policies.

The role offers a defined work schedule with salaried hours and opportunities to impact patient outcomes across Peterborough, GTA and surrounding areas.

Qualifications

• Licensed to practice in Ontario (or eligible for Ontario College of Pharmacists)

Responsibilities

• Act as liaison between long-term care homes, retirement communities and CareRx Pharmacy to ensure optimal resident care

• Provide education to residents and staff on medication topics
• Attend community-based Wellness events and professional meetings

CareRx is looking for a Clinical Consultant Pharmacist to join our team. TheClinical Consultant Pharmacistis responsible for maintaining continuity of care for clientele through collaboration between onsite physicians, nursing staff, and other health providers at long term care, supportive living, and other congregate living communities.You will be responsible for completing clinical pharmacy activities in alignment with the Continuing Care Health Service Standards, site policies, internal CareRx policies, and the Ministerial Order for Clinical Pharmacy Services.

Role Accountabilities:

• Act as aliaisonbetweenlong term care homesand retirement communitiesand CareRx Pharmacyto ensure optimal Resident care, Pharmacy Services, and Clinical support
• Travel to LTC homes and retirement communities
• Build and maintain relationships within the community to ensure ongoing collaboration and communication between home/communityleadership teams and pharmacy teams to develop/maintain strong partnerships
• Complete medication reviews in currently serviced retirementcommunities and long-term care homesserviced by CareRx Pharmacy
• Complete all required documentation and billing processes as required by MOHLTC, Alberta College of Pharmacy (ACP), and other federal and provincial standards and regulations
• Provide education to Residents and staff on various health-related topics including theMedsCheckprogram and attend community-based wellness fairsand other eventsas required
• Conducteducation sessions withinLong Term Care Homes and Retirement Communities withinscope of practice, as applicable (e.g. policy and procedure, web portal,eMAR, influenza protocol, etc.)
• Support CareRx Pharmacy policies and procedures within retirementcommunities and long-term care homes
• Completemedication audits on-site in LTC Homes and Retirement Communitiesas required
• Ensure regular on-site visits are made to all assigned clients
• Attend/participate in annual professional conferences and trade shows, when applicable
• Participate in the development and implementation of industry-leading clinical pharmacy service, when applicable
• Be an education, policy and pharmacy initiative resource
• Be available for special clinical team projects as required
• Complete/collaborate in the develop of medication audit follow-up improvement plans when applicable
• Works collaboratively with other team members to support applicable homes and communities
• Participate in pharmacy-related transition processes (e.gnew client transitions,eMARtransitions, etc.)
• Help with pharmacy operations/drug distribution when required
• Attend and actively participate in clinical team meetings

What You Will Bring to The Team:

• Insured and currently licensed in good standing with the Ontario College of Pharmacists, or College of Pharmacists of British Columbia (RPhT), or Alberta College of Pharmacy (ACP)
• Proven ability to foster strong partnerships with other members of the healthcare team
• Effective organization, planning, and time management skills
• Knowledge of Kroll Windows applicationand Microsoft Office programs
• Completeunderstanding ofprovincial regulations governing LTC homes and Retirement Communities
• Must be familiar with the provincial standards of practice established for LTC pharmacies
• Excellent customer service and interpersonal skills, oral and written communication skillsand be able to interact with persons of varying levels of professional expertise
• Excellent time management skills
• Skilled in critical thought processes and be able to prioritize appropriately
• Must demonstrate initiative and creativity

Compensation Range: $105,000 - $120,000. Actual pay rate will be based on the individual candidate’s credentials and experience.

Location:Location will primarily be in our LTC homes and retirement communities in the Peterborough, GTA and surrounding area

Work Shifts: Salaried position. Monday to Friday.
