Job summary

VHA Home HealthCare in Toronto is seeking a Client Safety Program Lead to drive organization-wide safety initiatives grounded in client safety science and high-reliability principles. The role partners with clinical leaders and operations to embed safety into daily practice.

You will develop and evaluate safety frameworks, lead incident reviews, and oversee Education and CIM processes, aligning with Accreditation Canada standards and organizational learning.

Qualifications

• Master’s degree in a health-related field (e.g. Quality and Patient Safety, Public Health, Health Administration) or equivalent.
• Regulated health professional preferred.
• Certified Professional in Patient Safety (CPPS) or equivalent certification.
• Minimum five (5) years of progressive experience in patient safety within a healthcare setting.
• Experience leading organizational patient safety programs and initiatives.
• Experience leading incident review processes and implementing systems-level improvements.
• Experience with Accreditation Canada readiness activities and safety-related practices.
• Strong knowledge of client safety science, safety culture, systems thinking, and high reliability principles.

Responsibilities

• Leads client safety program development, implementation, evaluation and continuous improvement.
• Establishes and monitors client safety priorities, goals, indicators and work plans.
• Provides organization-wide consultation on client safety including just culture and human factors.
• Leads client safety governance, including chairing the Client Safety Committee.
• Monitors client safety trends, identifies risks and opportunities, and improves safety.
• Oversees incident management and organizational learning from safety events.
• Develops and delivers client safety education and incident management processes.
• Coordinates readiness for Accreditation Canada’s Required Safety Practices.
