Job summary

C-Care Health Services in Toronto is seeking a Client Care Coordinator to support clients and staff through clear communication and care coordination. You will be the main contact for client health concerns and ensure care plans reflect changing needs.

The role requires an active CNO RN or RPN license, a valid BLS certificate and strong written and verbal communication. Prior care coordination and client management experience are assets, with on-site work in North York.

Qualifications

• Current and active CNO RN or RPN license.
• Valid BLS certificate.
• Good verbal and written communication skills.
• Previous experience in care coordination is an asset.
• Previous client management experience is an asset.

Responsibilities

• Conduct initial and ongoing in-home client assessments as needed for communicating services provided by the agency, updating of care plans, training of staff, etc.
• Create and update care plans for new and existing clients, and ensure that it is accurate and in accordance to the client's condition and needs
• Discuss the care plan to the staff and ensure that any changes or updates are being relayed to the appropriate staff in a timely manner
• Respond to client or family member inquiries relating to the client's care and condition
• Communicate with the client or family members any concerns, significant events, or scheduling changes that might impact the care being provided
• Monitor staff performance including but not limited to attendance, job performance, conduct, reporting, etc.
• Monitor shift notes and incident reports received from the staff and take appropriate action as needed
